With focusing on comfort care instead of curative care, we are looking at how to make the best out of every single day throughout the journey with end-of-life care. To do this, we take an interdisciplinary approach and provides a continuum of care to meet both patient’s and family’s medical, social, emotional, and spiritual needs.
Our Hospice Team understands that losing a loved one is a lifelong journey in which we want to walk beside you on. At Heartland Hospice, all disciplines within our team are available to every patient and family receiving our care. These disciplines include Skilled Nursing, Aide Services, Social Work, Spiritual Counseling, Bereavement Support, as well as Volunteers. Our Bereavement Support does not end at the time of your loved ones passing, rather it extends 13 months following their passing. We strive to navigate the journey of healing together with you by offering a space of comfort, understanding, and solace.
If you are not sure which services are right for you, give us a call and we will gladly help you navigate the options available to you.

Continuum of Care Provided

At Heartland, we are committed to meeting our patients and their families along every step of their healthcare journey and walking with them through their journey. To accomplish this, we provide a comprehensive continuum of care that seamlessly integrates Home Care, Home Health, Senior Living, and Hospice Care, allowing us to personalize care plans to meet individual needs and goals. Our holistic approach ensures that every person receives the right care, at the right time, in the right setting, tailored to their unique circumstances.

Hospice Care might be right for you after receiving a terminal diagnosis when you are ready to switch to comfort end-of-life care.
